USCIS I-693 Certified Exam
If you need to schedule an Immigration Physical with our USCIS certified Immigration Doctor, please call (303) 665-8444 and complete our digital paperwork. Immigration Exams must be scheduled over the phone.
You now have the option to complete a portion of your required South Pointe Healthcare paperwork online, instead of in person before your appointment. Filling out this form before your visit will speed up the time it takes us to process paperwork, and with current COVID-19 safety concerns it helps protect you by not having to spend as much time in our waiting room.

PLEASE NOTE: You will still need to fill out an official I-693 which can be found on the U.S. Citizenship and Immigration Services website. The I-693 as well as instructions on how to fill it out properly can be found here.

Vital Information For Your Immigration Appointment
Please read the below list of items to make sure that you are properly prepared for your Immigration Exam.
- You must bring a non-expired Passport or other form of Government issued identification.
- Bring any proof of vaccination history at your first appointment. Any delay in proof of vaccination history will delay your final exam report time.
- The exam consists of two separate appointments.
- The first appointment will last approximately 45 – 60 minutes, the second will be approximately 20 – 30 minutes.
- Payment is due at the first examination. We take cash and credit cards. Immigration Exams are not covered through insurance.
- The exam costs $250 plus the varying cost of additional labs.
- If you need any vaccination or titers to prove immunity, this will cost extra.
- You are not required to purchase these potential vaccinations / blood work at South Pointe Healthcare. You are welcome to shop around and/or see if your primary care provider carries the particular vaccinations that you need. For convenience, we offer similar prices on vaccinations compared to other local clinics.
- There are no refunds for services after they have been rendered. If a patient decides to discontinue treatment after it has been started , a full refund will not be issued. In individual circumstances a partial refund may be discussed with the practice manager for services that were not already performed by one of our providers.
Immigration Physical FAQ's
Immigration Physicals are the official physical examination for Green Card Registration.
The United States Citizenship and Immigration Services (USCIS) requires green card and U.S. citizenship applicants to receive a full physical.
A physical is required for citizenship and green card requests so that you can receive vaccinations for any diseases you would be at risk for in the United States. The exam also ensures you do not carry any diseases or conditions that would prevent you from obtaining citizenship.
Several vaccinations and tests are required during the immigrant physical, including:
- Syphilis blood test
- Gonorrhea urine test
- Influenza vaccine
- Measles vaccine
- Diphtheria vaccine
- Tetanus vaccine
- Pertussis vaccine
- Polio vaccine
- Mumps vaccine
- Rotavirus vaccine
- Rubella vaccine
- Hepatitis A and Hepatitis B vaccines
- Meningococcal vaccine
- Varicella vaccine
- Pneumococcal vaccine
If you have a copy of your vaccination records, we will be able to review them and determine if further vaccinations are needed. If you do not have your vaccination records, for an additional cost the Provider can take a blood test to prove that you have been immunized. Be sure to bring a valid form of ID so that we can verify your identity.
If you have any immunization records, whether American or from your home country, please bring them in. It is best to have them translated to English, if we are unable to interpret them, for an additional cost, we can take a blood test to prove that you have been immunized.
The full INS immigration exam requires two office visits as follows:
Your first appointment would be the Basic immigration physical exam and mental status evaluation (exam sometimes may be performed on the second visit). The first appointment will last approximately 40 minutes. Due to the complexity of the type of examination, our staff asks that we receive all of your examination paperwork prior to your first appointment to allow for adequate time to go over all of the needed information and vaccinations.
Your first appointment may also contain the following tests:
- Blood Test: Applicants age 15 and older are required to have a serologic blood test for HIV (AIDS) and Syphilis (RPR).
- Tuberculosis Skin Test: Applicants age 2 and older are required to have a tuberculin skin test (TST) performed. This test requires a return visit to the office within 48 – 72 hours after administration. If the test reading is greater than 5 mm of induration, a chest X-ray will be required. The cost of the X-ray is not included in the price of the immigration exam. This will be scheduled with a third party company and the cost is not part of South Pointe Healthcare’s fees.
- You will need Proof of vaccinations or blood Titer to prove immunity. Most adults require proof of vaccination for Tetanus-Diphtheria Td (within the last 5 years), Measles, Mumps, Rubella and Varicella (Chicken Pox). Blood Titer is a test that proves that a person has immunity or antibodies for a particular disease. The Immigration Doctor will review your vaccination history with you to determine whether you have all the required vaccinations. The cost of additional vaccinations is not included in the price of the basic exam but they are offered through our office for an additional fee. Please bring all vaccination records to your scheduled appointment for the immigration physical.
After the immigration physical examination is completed, the Doctor will certify Form I-693 with the test results of the immigration exam and vaccination history and place the same within a sealed envelope to be presented to the INS. DO NOT OPEN THE SEALED ENVELOPE. Turn in the envelope with your immigration application. A copy of your blood test results will be provided for you to keep after results have been discussed with the examining doctor. The examination will only be valid up to a year after the immigration exam date.
Your results will be presented to an immigration officer by either yourself or your representative. Your results may be shared with certain U.S. health departments. Please note that medical examinations are generally valid for 60 days.
Most health plans do not cover these services. We accept Cash, Checks, Major Credit Cards, and HSA cards that are back by a major Credit card company. We do not accept foreign payments, nor do we accept Care Credit for services rendered. All patients will pay South Pointe Healthcare directly and we can provide you with a receipt of services to bill your insurance to see if they will be willing to reimburse you for any of the costs involved.
Immigration Physicals start at $250, plus the additional cost of any needed vaccines. We do not bill insurance for immigration exams and vaccinations, however, an itemized receipt would be available for patients to submit to their insurance company for reimbursement if their insurance is able to do so.
